Shepherd's Hope Back to School Physicals

Back to School and Sports physicals at no cost for youth 18 and under without medical insurance. Parent or legal guardian must be present. What is covered: School physicals and Sports physicals, if needed (Participation forms must be provided by the parent or guardian). Your child must be 18 years old or younger; be uninsured and have no other insurance options. Parent or legal guardian must be present for a child to be seen by a physician and have any physical or immunization preformed.

How do I sign up? A parent or legal guardian must secure a "ticket" for each child through this Eventbrite registration. Once complete, the parent or legal guardian will receive an email through Docusign to complete the required registration paperwork.
